Welcome to Diana’s Faith House.  Diana’s Faith House is a non-profit, faith based, Shelter for women and women with children. Our goal is to help you become self-sufficient by assisting you with your job search, setting up a budget and savings account, and when it is time to exit the program, to help you find long term housing. We will also assist women with children, by getting them established in school. If you have any questions or concerns, please ask a staff member to help you. We are also here to mentor you through general life skills and spiritual enrichment.

 

You must actively pursue self-sufficiency.  This means finding a job.  It is up to you to find regular employment.  If you are unable to work, you must pursue alternatives such as schooling, job training, or state and/or federal assistance.  You may also work and pursue educational/career goals. Your progress in this area will be evaluated. All residents will be required to submit a daily activities log, which will be evaluated for “forward motion”.  All residents must leave the house between 10:00 a.m. and 1:00 p.m. 

 

We ask that you follow a set of rules while staying here. These are designed to make Diana’s Faith House a safe and healthy environment, and to help you and your children assume basic responsibilities.

Please understand that if you or your children refuse to follow these rules, we have the right to refuse to provide you services.

 

INTAKE:

You must disclose all information required during the intake process. This information must be accurate to the best or your knowledge. The nature of the information you provide will not be held against you, or prevent you from staying at Diana’s Faith House. However, withholding information, or providing false information, is grounds for immediate discharge.

 

Basic Guidelines for Intake Non-Acceptance

      1. If the person is under the influence of drugs or alcohol at intake attempt they are not eligible for an intake.

      2. If the public search done prior turns up recent and or history of violent crime or sex offense.

      3. The countenance of the individual is aggressive or uncooperative, and after the intake interview it is 

          concluded that they would be a detriment to the program or individuals/themselves.

 

Rules 1 through 10 are ‘zero-tolerance.’ This means if you break any of these rules, you will be discharged immediately from Diana’s Faith House.

 

1.  No drugs, alcohol or weapons are allowed on the premises.

 

a. Drugs include ALL over the counter and prescription medications. If you have medications that a doctor has prescribed, they are ALL TO BE SURRENDERED UPON INTAKE.  All over the counter medications must be surrendered upon intake also, except for inhalers, heart medication and Epi Pens. Medications will be kept in a locked area. Self-administration with supervision is at 9am and 9pm.  It is the Resident’s responsibility to tell the Supervisor when they need to take their medication(s).

 

 b. No alcoholic beverages may be consumed while participating in the Faith House    

 program. This includes ALL alcoholic beverages. If you smell of alcohol you will be  

 tested. No mouthwash containing alcohol is allowed.

 

             c. Weapons include ALL guns, pocket knives, utility knives, box cutters, scissors, etc.

 

       d. Random alcohol and drug testing may be administered. Refusal of random testing                                                                        is grounds for immediate discharge.

 

2. No pornographic material on the premises. Diana’s Faith House reserves the right to

            define what is or is not ‘pornographic’. If you have questions regarding this, consult the

            staff. A good rule of thumb is that if it is not “G” it’s probably not allowed. Television

            programs will be monitored for appropriateness and violent content.  

 

3. No theft of property whether it is Diana’s Faith Houses property, or that of other residents, etc. In the

    event of theft, charges will be filed with local law enforcement and you will be immediately discharged.

 

4.  No photography or videoing of any kind in shelter.  Safety of those in shelter is paramount.  Do

     not disclose whereabouts of persons in shelter purposefully or accidentally, especially on any type of 

     social media (Facebook, Snap Chat, YouTube, etc.) or by use of any other type of photography or

     videography.

 

        5. No physical abuse whether staff, volunteers, residents or visitors. Physical abuse includes shoving, pushing, finger-poking, and touching inappropriately. In the event of physical abuse, charges will be filed with local law enforcement.         

 

        6. No negative involvement with the police. This means being arrested, cited or warned for any reason. This does not mean being stopped and/or questioned.

 

        7.  No swearing, foul language or sexual comments. This includes lewd, obscene or “off

             color” jokes or stories.

 

        8.  No verbal abuse or harassment of staff, volunteers, residents, neighbors or visitors.

             This rule includes lying, spreading of rumors and general negativity toward the program.

        Discussing other residents outside of the Shelter staff is not permitted.  Every resident is

        entitled to confidentiality.

 

   9.  You must be in by curfew unless you’re working or have prior permission from the House Supervisor.

         Curfew is 7 p.m. 

 

   10. All residents physically able to work will be required to fill out a job contact sheet weekly and

          turn it in each Monday morning. All able bodied residents will be required to be working by the end

          of their first 30 days. Twenty job contacts per week are required and must be verifiable. Children will

          be placed in school and/or daycare during job search.

 

Rules 10 through 33 are ‘three strikes and you’re out.’ This means you will receive a verbal warning on the first infraction. On the second infraction; a write-up.  On the third infraction, you will be discharged from the Faith House program. Unless of course you are on zero tolerance…

 

10.   You must sign in and out when leaving and returning to the house.

 

11.   If you are in the shelter, you are expected to attend meetings.

 

12.   You must complete your assigned house chores on time. If your regular work schedule prevents you from completing your assigned chore, you must discuss this with the House Supervisor and find someone to trade with you. Finding a replacement for house chores is your responsibility. Children may be assigned a chore depending on their age and ability, parents will have the responsibility of monitoring the child’s chore completion.

 

13.   You must complete 3 activities per week. If you have alcohol or drug issues, AA/NA programs will be mandatory. The Life Skills class is mandatory. Optional activities include Church Services and Bible Study & other classes on and off-site. Off-site classes must have House Supervisors approval prior to attending classes.

 

14.   If you are recovering from drug or alcohol addiction you may be required to attend ’30 in 30’ –that is, 30 sessions in 30 days, of either Alcoholics Anonymous or Narcotics Anonymous, or up to 90 in 90, as decided by administration.

 

15.   No panhandling anywhere while you are in any Faith House program.

 

16.   No shopping carts are allowed on the premises.

 

17.   No loitering or around the property of our neighbors. Residents who wish to relax outdoors may do so in the designated area behind the house.  NO SMOKING IN FRONT OF THE HOUSE.

 

18.   SMOKING: You are strongly encouraged to quit smoking for your health and the expense.  It is difficult to explain to donors that folks who stay here are homeless, but they can afford cigarettes.  If you must smoke, use the designated smoking area around the backside of the premises.  NO SMOKING ALLOWED ANYWHERE ELSE IN OR ON THE PREMISES.

19.   You and your children must shower & brush your teeth every day. Supplies such as soap and shampoo are provided free charge if you are not working. Residents who do not maintain proper personal hygiene may be required to shower immediately and dress in clean clothes, etc. Failure to do so may result in immediate dismissal from the program.

 

20.   You must launder your clothes and bedding regularly. House laundry will be completed as set in the daily schedule. The family laundry schedule will be posted weekly.  You must purchase your own laundry supplies if employed.

 

21.   Sleeping is only allowed in your room. No sleeping in the living room.  

 

22.   No food or beverages in your room. Residents must eat and drink in the dining

        room area only.  Water bottles only are allowed in the bedrooms.

 

23.   Proper attire must be worn at all times, including when going from the shower to

        your room. This means shirt, pants or shorts, and shoes. Proper sleep clothing

        must be worn when sleeping. We suggest you wear shower slippers when showering.

 

24.   Clothing with objectionable logos/language may not be worn.

        Objectionable themes include anything obscene, vulgar, demonic, drug, alcohol or

        sexual related. Clothing includes shirts, vests, pants, belts, sweaters, jackets, coats, hats and scarf’s.

 

We have access to the clothing closet at Cornerstone Mission. Please let staff know if there is a need and what that need is. We will do all we can to assist you.

 

25.   Visitors are allowed but permission must be given prior to the visit. All visitors

 are subject to screening which may include a criminal history check. Visits should be brief   to be respectful to the other residents and the shelter classes/activities.

 

26.   Visitors are not allowed in the bedrooms and must use front restroom.

 

27.   Residents personal belongings must be kept either in the closet, your dresser drawer or your tote under the bed. Items left on the floor may be thrown away. Rooms must be picked up before leaving for the day. Closets must be kept orderly.

 

28.   CD players and other audio gear may be used only with headphones. Phones must be silenced or placed on vibrate at 10:00 p.m. Non compliance will cause confiscation of the

        equipment which will be held by the House Supervisor.

 

29.   Respecting Boundaries.  You may not go in another residents room and/or property                                                                          

with out their permission. You may not borrow any other resident’s belongings without their  permission.

 

30.   Residents are not to fraternize with volunteers, staff or clients on a personal level

        other than polite conversation. Residents are not to ask any volunteer or staff for

        money. Needs may be conveyed to staff and necessary financial help may be warranted but

        must be determined by administrative staff.

 

31.   Children and discipline.  Time out’s and restrictions are the preferred method of discipline within the shelter. There will be no physical, verbal, mental or emotional abuse tolerated.

 

32.   Children must be supervised if playing in the backyard. Residents are allowed to take turns doing so.

 

33.   Residents are not allowed to discipline other residents children.

 

Residents must be aware of the fact that we are many families living under one roof. We

have different personalities, beliefs, likes and dislikes. We must respect each other in order

live peaceably under one roof.

 

Diana’s Faith House strives to maintain a safe and healthy environment for its residents. We also abide by rules and regulations imposed by various state and local agencies. To this end, Faith House reserves the following rights:

 

We have the right to search all residents and their personal belongings on house/shelter property at any time, without prior notice. This includes lockers, backpacks, satchels, purses and any other container that holds your belongings. If you refuse the search, you will be discharged immediately. If the search produces contraband, including drugs, alcohol, weapons and/or pornography, you will be discharged immediately.

 

We have the right to test for drugs and alcohol at any time, without prior notice. If you refuse the test, you will be discharged immediately. If the test result is positive, you will be discharged immediately.

 

DISCHARGE ISSUES:

If you have been discharged for non-compliance with the rules, and are refused shelter, you may appeal the decision. The process is as follows:

 

Within 72 hours you must submit a written appeal to the Administrator of Cornerstone Mission. The Administrator must respond to your appeal within 72 hours of receiving it. If the Administrator rejects your appeal, you may submit a second appeal to the Executive Board of Directors within 72 hours of the Administrator’s rejection. The Executive Board must respond to your appeal within 72 hours of receiving it. Their decision is final.

 

 

Whether or not you stay at the facility while your appeal is in process, is a decision the Administrator will make. It may not be appealed.

EXITING THE PROGRAM:

Upon your exit, all medications and personal property being held for you will be returned.

Additionally, any expenses paid in advance will be refunded to you. If you’re discharged for non-compliance of rules, you will not be refunded shelter obligation paid in advance.

 

Upon exit, your belongings may be held for you up to 5 days only. After 5 days, they will be disposed of in any manner which Diana’s Faith House/Cornerstone Mission Project, Inc. sees fit, including the sale of, donation of or destruction of said items.

 

If a resident leaves the program before the 90 days she will not be allowed back in the program for 30 days. Any returnee will be on zero tolerance. NO three strikes.

 

RESIDENT STATUS:

During the first 7 nights of your stay at Diana’s Faith House, you are being evaluated, and are housed at no charge.

 

Once you are receiving regular income, it is mandatory that you open bank account and develop a budget. Staff will guide you through this process. You will be required to complete 2 activities per week.

 

You will meet with the Case Manager at least once a week. Participate in the development of a plan towards self-sufficiency. Apply for and follow through with community resources available to you.  Apply for mainstream resources, such as food stamps, ACHHS, subsidized housing, etc.  The Case Manager will help you get to self-sufficiency, the timeline is up to you.

 

Residents may be eligible for an overnight pass after staying for 30 consecutive days. Any earned passes should be discussed with shelter staff for approval in advance. In addition shelter staff may offer overnight passes at various times and Holidays.  You must be current on your shelter obligations. Passes are granted on an individual merit basis.

 

Length of stay at DFH is limited to 90 days. You may apply in writing for an extension. You will need the recommendation of your forward motion from your Case Manager.  Extensions will only be granted by administration.

 

Diana's Faith House staff, volunteers and guests are expected to treat each other with mutual respect. If a resident or guest encounters a situation or condition which they believe affects them unjustly or inequitably, they should speak with the Shelter Supervisor or their Case Manager. If the situation or condition cannot be satisfactorily resolved by the staff person, the resident or guest may contact the Executive Director.  Should the Executive Director and the resident or guest be unable to resolve the issue, the guest may contact CMP Board of Directors. The CMP Board of directors should only be contacted directly when the guest feels they have tried multiple times to resolve an issue and have been unsuccessful.

 

SHELTER FEE/OBLIGATION:

All residents shall be responsible for a shelter obligation of $25.00 per week per adult.   No charge for children.  If you are not working steadily, you may use your food stamps to pay your fees.  Please see the House Supervisor for a list of needed items for the house.  Purchase $25.00 in groceries with your food stamps and return the receipt and the food to the House Supervisor.

 

You may also be asked to help with groceries from time to time.  All food provided to Diana’s Faith House comes from St. Mary’s Food Bank in Phoenix and items the public donates.

 

HOUSE HOURS:

WAKE UP: -  All residents must wake up early enough to get themselves (and their children if any) ready for the day and complete their morning chores by 9:00 a.m..  Residents may set their own wake up alarm as long as they act responsibly and are up within reason. You must be aware of the shower and laundry schedules.  Saturday is a “free” day and you are allowed to “sleep” in until 8:00 a.m. (as long as your children are asleep also). Under certain circumstances, and with prior permission, you may be allowed to remain at home during the day.

 

Monday through Friday the TV may be turned on at 3:00 p.m. and not before.

 

LIGHTS OUT:

“Lights out” for children is 7:30 p.m. Sunday through Thursday and 8:30 p.m. on Friday, Saturday and holidays.

 

“Lights out” for teenagers is on a case by case basis with parental input.

 

‘Lights out’ for adults is 10:00 p.m. Sunday through Thursday and 11:00 p.m. Friday and Saturday.

 

We would prefer that other residents not be given the responsibility of watching your children; however in certain circumstances, there may be no other choice. Please understand that we are not a day care facility. If in an extreme situation, you have no other option but to ask another resident to watch your children, or if you are asked in an extreme situation to watch another resident’s children, there will be no exchange of money for this. You must check with the House Supervisor before asking another resident to care for your children in your absence. Staff will not be given the responsibility of caring for your children. 

 

VEHICLES & BICYCLES:

If you have an automobile, you must obtain permission from the administrator to park it on the premises. The vehicle must have current registration and insurance, and you must have a valid driver’s license and proof of ownership. Vehicles must be in working condition. There will be no storing of non working vehicles on the premises.

 

If you have a bicycle, you must keep it locked up in the back yard.

 

Automobiles and bicycles may not be parked or left unattended off Faith House property. You may not park in front of our neighbor’s homes or vacant lots in the area.

 

Times and dates for House Meetings will be posted.  All residents are required to attend meetings. It is your responsibility to make sure no other appointments conflict with these meetings. You are expected to attend if you are not at work. House meetings may be called without notice if the need arises.  If you feel you are unable to attend for some other reason, you must consult staff. Meetings addressing spiritual or religious issues are not mandatory. 

 

ADDITIONAL GUIDELINES:

From time to time when we need additional help, and you are in the facility, we may ask for your help beyond normal assigned chores. Failure to be accommodating when asked for help by staff will be noted in the resident’s file.

 

MEALS:

You will be responsible for providing your own breakfast and lunch. Dinner will be served family style daily at 5:30 p.m.  Meal times and a dinner menu will be posted.

 

Residents will all take turns cooking and have kitchen responsibility. If you have favorite dishes that you would like to prepare please let staff know.

 

If you require a special diet for medical or religious reasons, you must notify the staff during intake. We will make every effort to accommodate these needs.

 

RESIDENTS WITH FAMILIES/VISITING FATHERS:

It is our goal at Faith House to keep the family unit together as much as possible. For this reason we allow fathers and boy’s age 13 and above to visit.

 

The guidelines are as follows:

 

Prior approval must be obtained and staff must be made aware of any visitor’s presence.

They may join us for dinner nightly.

They may stay until 7:00 p.m.

They are not go into any bedrooms.

Visitors must use the front restroom.

Please refrain from public displays of affection.

If you or your visitor/partner argue or fight in front of or in hearing distance from any resident, staff or children, this privilege will be taken away.

 

KEEP IN MIND:

It may be in your best interest to use courtesy liberally during your stay with us.  For instance, if you will not be having dinner at the designated time, with the rest of the house, let the House Supervisor know early so that dinner is not prepared for you.  If you aren’t sure about something, ask.  If it is not specifically in the rules, ask.  We can always add rules.

 

Services are provided to you through the donations of individuals and organizations in this community who partner with us to help you as well as the wonderful volunteers we have.  We are very blessed to have their support and fully intend to be good stewards of their donations and time.
